Food-contact surface soiled with food debris, mold-like substance or slime. Observed : the can opener blade has dried food debris on it. This was moved to dish. Observed : the ice machine shroud edge has dark foreign substance on it.
Raw/undercooked animal food offered and establishment has no written consumer advisory. Raw animal foods must be fully cooked prior to service. Observed : the newest version of the menu lacks a consumer advisory. A consumer advisory was placed at the front door, staff will address at the point of order, manager will make return address size labels to add to the menu.

Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Operator stated dish machine had not been used for a few hours but after running dishwasher machine twice and priming sanitizer line was still not dispensing properly.
Dead roaches on premises. Underneath back storage shelf by back door. Operator cleaned area.
Equipment in poor repair. Chest reach in freezer lid is not smooth easily cleanable. Food shelving in walk in cooler the paint is peeling and shelves starting to rust.
