Heat strip failed to turn black to indicate the sanitization temperature of 160 degrees Fahrenheit was achieved on the dish surface and/or high temperature holding thermometer did not reach 160F. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired. Heat strip did not change color. Triple sink set up to sanitize. Do not use dishmachine until it can properly sanitize. Ran it twice.
Water treatment device has not been inspected or serviced according to manufacturer's instructions. Filter at servers station should have been changed 12/23.

Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee went outside to grab to go trays, the employee came back into to the establishment and put on clean gloves no hand wash, he then went back to engaging in food preparation.
