Handwash sink used for purposes other than handwashing. Observed a scrubby and scraper in the hand washing sink in kitchen. Operator removed items from hand wash sink.
Food stored on floor. Observed a case of bread and a case of chicken on the floor in the walk-in freezer. Operator moved items to a shelf.

Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed hose Bibb at m sink outside next to back door without a vacuum breaker.
Non-pitting surface rust on food-contact equipment. Observed the bottom surface rust on the bottom shelf of food preservation table in back kitchen.
Unwrapped single-service utensils not presented so that only the handles are touched. Observed single serve forks not presented handles first. Operator turned all forks the same way to present handles first.
