Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed on the cook line and the bar: Colelsaw (60F - Cold Holding); Chopped tomatoes (53F - Cold Holding); Cheese olives (62F - Cold Holding); Chopped watermelon (56F - Cold Holding) Operator stated they had been out for approximately 3 and a half hrs. Operator discarded the cook line foods of their own decision and placed the bar food in reach in coolers.
Bowl or other container with no handle used to dispense food. Observed bowls being used as scoops for dry goods. Operator removed them.

Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Observed wiping cloth sanitizer to strong. Operator corrected lactic acid sanitizer to 700ppm.
Ceiling/ceiling tile shows damage or is in disrepair. Observed after dripping from ceiling in server area. Operator stated that they have had issues with the roof.
Covered waste receptacle not provided in women's bathroom. Observed garbage in unisex rest room for employees with out a cover.

Operator was unable access information in computer to provide proof of any training.
Cove molding at floor/wall juncture broken/missing. Observed multiple areas of cove molding through our kitchen broken and pulling away from wall.

Manager moved tuna to bottom shelf. Raw pork stored over flour tortillas in reach-in cooler on left side of cooks line. Manager moved raw pork to bottom shelf.

Manager stated the items had been stored in reach in cooler for approximately 1.5 hours. Manager emptied half the content of the pan into separate pan and placed it in the bottom of the reach in cooler.

Operator stated walk in cooler was approximately 36F at approximately 9:30 am. Advised operator to open walk in freezer and add ice to TCS foods. Operator placed call to repair company.
