Observed employee remove ear bud from ear while prepping dressings and then put back in ear and continued to prep dressings without washing hands in between and put on gloves. Discussed with operator.
Live, small flying insects found. Observed one flying insect on floor in beverage station.

Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed ready house made salads (cut lettuce) hard boiled eggs, sliced cheddar cheese (47F-51F - Cold Holding) in reach-in cooler at front service line. Foods were in cooler overnight. See Stop Sale.

Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed interior of large ice machine soiled with mold like substance.
Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Observed two ice buckets at large ice machine not protected/ inverted. Operator inverted both ice buckets.
Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Observed reach-in cooler at front line used for salads ambient temperature 50F. Observed ice build up on fan in back of cooler. Also, walk-in-cooler with meat ambient 58F. Operator stated it was in defrost. Operator called to have serviced. Retemped 15 min later at 55F.

Observed approximately 27 dead roaches in the following areas: 7 under food warming boxes near AC closet 2 on floor under hand wash sink in smoker room 3 on floor under rolling bread racks 1 on shelf below front counter 1 on lid of box of to go bags at front line area 6 on floor in front service line area 2 on floor next to deep freezer near prep area 1 in tray of warm towel box 1 on top of dishwashing machine 3 on floor under dishwashing machine
