No soap provided at handwash sink. Observed no soap at hand wash sink in bar area. Operator replenished soap.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed employees personal belongings stored on counter next to ice bin and also in back cabinet on top of single use towels. Employees removed cell phone and food to properly store.
Plumbing system in disrepair. Observed one hand wash sink at bar with sign on it, out of order. Observed hand wash sink not working. Operator stated it's been called in to have service. Bar did have one other operating hand wash sink.
Reach-in cooler interior/shelves have accumulation of soil residues. Observed Interior of reach-in cooler with bottled waters soiled on bottom.

Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Sanitizer Bucket (Quaternary 500ppm) under front counter. Operator had employee remake solution and retested at 200ppm
Single-service articles not stored inverted or protected from contamination. Stirrers on front bar counter not protected from guests coming to counter to pay. Operator removed stirrers to protect stirrers.
