Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In the pizza station, cut tomato (54F - Cold Holding); tuna (52F - Cold Holding); cooked mushroom (51F - Cold Holding); shredded cheese (50F - Cold Holding). Operator stated foods had been placed on the line for 40 minutes. Operator removed foods and placed them in the walk in cooler.
Handwash sink used for purposes other than handwashing. Observed soiled dishes inside The handwashing sink across from 5e cook line in the main kitchen. Open removed soiled dishes out of the handwashing sink.

In Walk in cooler, observed cooked frozen beef with no date marking.
