Exterior door has a gap at the threshold that opens to the outside. Observed gab at bottom right hand side of kitchen door.

Outer openings not protected with self-closing doors. Observed self closing device on exterior of door not closing tightly.
Ripped/worn tin foil used as shelf cover. Observed bottom shelf of prep table with torn tin foil being used as shelf cover.
Wet wiping cloths used for occasional spills on equipment food- and nonfood-contact surfaces not clean. Observed on cutting board to flip top cooler in between both freezers in kitchen a wet cloth. Employee removed cloth.
