Quaternary ammonium sanitizer in spray bottle not at proper minimum strength. Quaternary sanitizer bucket at 0 PPM. Employee replenished quaternary sanitizer with new solution and chemical is now testing at 200 PPM.
Raw animal food stored over/not properly separated from ready-to-eat food. A case of raw shelled eggs was stored on the top rack above containers labeled bananas, coconut cake, sprinkles cake, and cases of milk in the walk-in cooler. On the opposite rack, an open case of raw shelled eggs was stored above butter packs. The employee moved both cases of raw shelled eggs to the bottom rack for proper storage.
Food-contact surface soiled with food debris, mold-like substance or slime. Mixer head soiled with dried up food debris.
