Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee return to truck from outside delivery wearing gloves. Changed into new gloves, no hand washing sign Rare. Operator stopped employee amend made them wash hands prior to putting on new gloves.
No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Operator retrieved sanitizer test strips from office to refill.
Bowl or other container with no handle used to dispense food. Observed plastic ramekin used as scoop in bread crumbs. Operator removed and replaced with handled scoop.
