Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed in reach in cooler near steam table Diced tomatoes (45F - Cold Holding); shredded cheese (45F - Cold Holding); cut cabbage (45F - Cold Holding). Cooler has an ambient temperature of 42F. Manager will relocate items inside cooler. Manager stated may be due to the lid being open during lunch.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed splitter added to mop sink with back flow preventer incorrectly installed before the splitter, the non chemical side of splitter needed back flow device. Manager placed a back flow on non chemical side of splitter.

Observed chemical dish machine at 0ppm. Chemical that was hooked up was empty. Manager was able to prime and get chemical dish machine to sanitize properly. Took second test at 100ppm.

Observed open container near coffee machine without lid or straw. Observed multiple drinks stored on prep shelves throughout kitchen. Manager removed and stored drinks properly.
