Chemical test kit not used to ensure proper sanitization of equipment and utensils when using a chemical sanitizer. Operator has chlorine testing strips and chlorine sanitizer but is using steramine sanitizer and does not have testing strips for quaternary.

Evidence of mop/cleaning wastewater dumped onto ground. Mop sink being used as dump sink for food waste and mop water being dumped outside. Educated operator and will be corrected from this point forward.
Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Milk opened on 10-20-25 not date marked.
Food-contact surface soiled with food debris, mold-like substance or slime. Reach in cooler under fruit has food debris on bottom shelf.
Cove molding at floor/wall juncture broken/missing. Back of store by walk in cooler.
Equipment in poor repair. Reach in freezer, tall glass door, has torn gasket and missing handle.
Insect control device installed over food preparation area. Rack with icing stored under bug zapper, operator will relocate food items.
Unwashed fruits/vegetables stored with ready-to-eat food. Bananas unwashed with stickers being sliced into ice cream ready to eat serving.
