Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Make bar- ambient 62F- sausage links 52F, sliced diced ham 58F, hash browns 34F, pulled eggs 56F (9:00-10:15). Held on ice- shell eggs 57F, pulled eggs 56F, shredded cheese 52F (9:00-10:14) employee replaced ice and placed lids over food. Equipment was unplugged, employee placed all food on ice and plugged it back in.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance through out kitchen.
In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Scoop handle stored in flour.
Multiple Soiled dry wiping cloths in use and stored on cutting boards, under flat top and through out kitchen on multiple food contact surfaces.

Exterior door has a gap at the threshold that opens to the outside near walk in cooler.

Plumbing system in disrepair. Hot water faucet does not turn on water at 3 compartment dump sink at front counter bar. Hot Water was turned off by establishment. Operator turned faucet back on.
Ten (10) foot-candles of light at least 30 inches from floor not provided in dry food storage area room near havoc.
