Observed raw shell eggs being stored over raw beef in kitchen reach in cooler. Advised manager that all foods must be separated from each other in holding unit based upon minimum required cooking temperature. Manager removed eggs.
Observed paper towels for employees to dry hands in kitchen area not accessible due to dispenser in disrepair. Manager added paper towels temporarily and created work order.
Observed Ceiling tiles and AC vents soiled with accumulated black buildup.
Observed crates of buns not stored at least 6 inches off of the floor. In prep/ ware wash area. Manager instructed employee to place buns on rack.

Observed reach in cooler gaskets soiled with food debris. Manager stated that new gaskets were ordered but they are awaiting delivery.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed the following temperatures greater than 41°F; Raw wagoo beef (69F - Cold Holding); raw angus beef (67F - Cold Holding) All were located in reach in cooler. Cooler had an ambient of 69°F and in disrepair at time of inspection. Advised that no food is to be stored in cooler until ambient temperature is +/-41°F. All food was discarded. Stop sale issue.
Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Observed reach in cooler with ambient temperature of 69°F. All food inside was discarded and manager called maintenance to repair. Al other reach in coolers in kitchen area in working condition.

Observed walk in cooler with ambient temperature of 48°F due to maintenance. Technician on site at time of inspection and stated he is currently working on issue and that cooler should be back up and running in the next couple minutes. All food stored in walk in cooler temperature was 41 or below.
