Time/temperature control for safety food held using a 4-hour time control plan was not at a temperature of 41 degrees Fahrenheit or below or 135 degrees Fahrenheit or above when time control began. Observed cut lettuce 72F per manager put out on line approximately 40 minutes ago. Also sliced tomatoes 70F put on line approximately 40 minutes ago. Manager placed cut lettuce and sliced tomatoes in walk in cooler and replaced with cut lettuce 37F and sliced tomatoes 38F from walk in cooler.

No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at any of the hand washing sinks. Manager stated waiting on truck delivery, using napkins from front counter to dry hands.
