Equipment and utensils not properly air-dried - wet nesting. Observed stack of pans on shelves with water in between, not air drying properly-operator moved.
In-use utensil for non-time/temperature control for safety food not stored in a clean, protected location. Observed tongs in a soiled pan in the reach in freezer in the kitchen, operator moved.

Ceiling tile missing. Observed small portion of a ceiling tile is missing leaving a hole, above soda boxes in back of kitchen, operator stated having trouble keeping it covered due pipes above it.
Employee with no hair restraint while engaging in food preparation. Observed employee with out a hair restraint on, operator had her put one on.
Food stored on floor. Observed a box of Hershey pies on the floor in the walk in freezer, operator moved to shelve.

Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed employee handle raw meat touching tongs that had been stored inside tray that had held raw meat then loading raw burgers into oven then without changing gloves and washing hands employee handled clean dishes and set up clean warmer trays that will be touched with clean gloves while directly handling ready to eat foods. Cross contamination possible from meat tray to tongs to gloves to clean warmer trays handles to ready to eat foods. Employee should after loading raw meat into oven drop off raw meat utensils then remove gloves and wash hands then add clean gloves before handling ready to eat. Manager addressed issue and retrained employee.
Equipment in poor repair. Gasket on walk-in freezer broken causing ice buildup in freezer.
Floor soiled/has accumulation of debris. Debris in storage area on floor under shelving.
Stored food not covered. Observed box of biscuits open and uncovered in walk-in freezer. Manager covered.
