Exit door locked. For reporting purposes only. Backdoor leading to the dumarea door is lock and it's hard to unlock the locking mechanism to open the door. Can be a fired hazard

Observed drive through beverage station ice bin interior is soiled around the edges with black like substance. Operator clean the interior of edges of ice bin

Ceiling/ceiling tile shows damage or is in disrepair. Observed in make bathroom there a hole in the ceiling. Operator stated that maintenance man is repairing it this afternoon
Hole in or other damage to wall. Observed wall at backdoor leading outside is in disrepair.
Wet mop not stored in a manner to allow the mop to dry. Observed mop stored in ringer at mop bucket not able to dry properly

Ceiling tile missing. Multiple areas were ceiling tile removed for the beverage and computer lines run thru location.
Hole in or other damage to wall. Small holes in walls thru location from moved equipment that needs filled in.
Unsealed concrete floor in food preparation, food storage, warewashing area or bathroom. Mop sink area needs sealed perimeter.
