Outer openings not protected with self-closing doors. Observed exterior door unable to self-close at entry ware facing Edgewood. Person in charge stated work order has been placed to repair and is marked off to not open.

Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed splitter with hose attached on non-chemical side at mop sink missing a vacuum breaker. Person in charge swapped hose and chemical-dispensing hose to proper locations with vacuum breakers.
Dead roaches on premises. Observed one dead roach under shelving in dry storage area. Person in charge cleaned area and disposed of dead roach.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed employee cell phone on prep table on backside of condiment station. Person in charge removed cell phone.
Employee with no hair restraint while engaging in food preparation. Observed; one employee at to-go window with no hair restraint engaging in food preparation. One employee on cook line with no hair restraint engaging in food predation. Person in charge provided hair restraints for both employees.
Water leaking from pipe and/or faucet/handle. Observed hot water dial leaking water at hand wash sink near to-go window across from office. Person in charge stated maintenance was aware and is in process of fixing.
Wet mop not stored in a manner to allow the mop to dry. Observed wet mop stored in mop bucket not-in-use with visible moisture and unable to air dry. Person in charge hung mop inverted on wall.

Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Under front counter at cash registers, Sanitizer Bucket (Quaternary 0ppm, retest 150ppm). Operator remade to correct.
