Sanitizer bucket stored with food, equipment, utensils, linens, single-service, or single-use articles. Observed : quaternary ammonia sanitizer bucket(200 ppm) stored on the lower, solid shelf with food, on the cook line. The Sani bucket was placed in a non cracked larger pan.
Storage of maintenance equipment/tools in areas that may result in cross contamination of food, equipment, utensils, linens and/or single-service and single-use articles. Observed : on the bulk soda rack, soiled heat resistant gloves and fry oil connecting tubes stored on the boxed sodas.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed: a quart size light cream carton in a reach-in cooler near the drive thru, 50F. Creamer inside was at 39F. The light creamer was opened during breakfast.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed the reach-in freezer on the cook line has an accumulation of food debris on the bottom level.
Equipment in poor repair. Observed: the in use bun toaster does not let intermittent buns pass through, and they come out with burnt edges.
Stored food not covered. Observed: white cheese stored in a reach-in cooler, cut open and not wrapped. Staff plastic wrapped the cheese block.
Toilet/urinal not flushing/functioning properly. Observed: the urinal was to the point of overflowing.
