Employee pulled pants up then proceeded to touch single service items. Coached operator on proper hand washing procedures.

Employee rubbed hands together for less than 10-15 seconds while washing hands. Cook went outside came in to wash, rinsed hands, then dried, total time 5 seconds, discussed with staff and manager on duty. Operator washed hands again.
Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. In vertical freezer on cook line- Raw hamburger patties, open box, over churros, cheesy tots, operator moved beef to bottom Single door freezer on cook line- raw chicken in open pan over fish, operator moved fish to bottom
Raw fruits/vegetables not washed prior to preparation. Onions cut before washing, operator put onions in tub and washed

Observed employee touch nose with gloves on while preparing sandwich then continued making sandwich without changing gloves or washing hands. Manager had employee change gloves and wash hands
Ice scoop handle in contact with ice. Manager removed scoop and put in dish area to be cleaned
