Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed deli ham and cut lettuce at temperatures of 45-47F in walk-in cooler. Walk-in cooler door was propped open 45 minutes prior and was not shut all the way. Observed pans of cut lettuce and cut tomatoes held on time as a public health control with no time marker on main cook line. Person in charge stated items were all placed on shelves at same time and employee added time marking.

Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Employee clocked in to work, put personal items on rack and when straight to front counter and began bagging to go orders without washing hands. Manager coached employee on proper handwashing procedures.
Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. In stand up reach in freezer on cook line, open bags of uncooked chicken over top of open bags of ready to eat French fries and onion rings. Employee moved all chicken to bottom shelf and ready to eat food to top shelf. Also, in reach in freezer next to fryers, open container of raw liquid egg stored on top of open bags of French toast sticks and hash browns. Employee removed liquid eggs from freezer.

Employee eating in a food preparation or other restricted area. On arrival to establishment, employee working drive through window was chewing food while putting packaged food into to go bag.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. On counter in dry storage room, employee cellphones on top of single service cup holders. Manager removed cellphones.
Equipment in poor repair. Reach in freezer across from mop sink in poor repair with insulation showing on top ledges where lid shuts.
Ice buildup in reach-in freezer and/or walk-in freezer. In reach in freezer across from mop sink build up of ice inside. Also, in walk-in freezer ice build up on floor near back wall.
Missing drain plug at dumpster. On dumpster out back of establishment missing drain plug.
Old labels stuck to food containers after cleaning. In back drive through area, clean pans with old labels still on them.
