Live, small flying insects found In kitchen approximately 1 live flying insect flying around drive thru expo area. In customer lobby approximately 6 live flying insects at soda station landing on drain and walls. Discussed with operator to kill all flying insects and clean and sanitize areas.
Presence of insects, rodents, or other pests. In kitchen on top of heat lamp over sand which station, approximately 20 dead ants, discussed with operator who cleaned and sanitized area.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. In hot holding unit at sandwich assembly station cooked eggs (106F - Hot Holding) per operator product prepared approximately 45 minutes ago and hot held until served, discussed with operator who moved product to reheat to 165F+.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Exterior Hose Bibb in front of store no vacuum breaker installed, discussed with operator to install vacuum breaker.
Dead roaches on premises. In kitchen approximately 1 dead roach on floor in dry storage, discussed its operator who cleaned and sanitized area.
