Creamers containing milk in front counter area: creamer (61F - Cold Holding) Discussed with general and assistant manager container is overfilled not allowing creamers at the top to be cold held. Manager put item into reach in freezer to bring down temperature.
Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Emailed DBPR Clean-Up of Vomiting and Diarrheal Events form. Establishment does not have chemical bleach onsite, as confirmed by the District Manager. Inspector discussed according to written procedures, an appropriate cleaning chemical must be available to ensure employees can properly respond to vomiting or diarrheal events. Form will be used as a template for establishment.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Lime scale build up observed in ice machine interior.

Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Mop sink has a splitter on it and no vacuum breaker.
Open dumpster lid. Manager closed lid.
Raw fruits/vegetables not washed prior to preparation. Employee cutting onions without washing them prior. Manager instructed employee to wash onions prior to cutting.
