Food-contact surface soiled with food debris, mold-like substance or slime. Observed sliding doors deep reach in freezer where raw hamburgers are stored with the bottom soiled with food debris.
Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Observed reach in cooler in front counter with ambient temperature of 60F. As per operator, awaiting service of the unit. Operator removed all products and placed into the walk in cooler.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed reach in freezer with soiled gaskets.
Observed ice buildup in deep reach-in freezer, by the kitchen. Observed walk in freezer door with ice buildup, and blocks of ice on the floor.
