Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. No written procedures available. Provided DBPR HR 5030-104.
No paper towels or mechanical hand drying device provided at handwash sink. Mechanical drying device at main sink in women's restroom is not working. Manager placed paper towels in the restroom.
No proof of required state approved employee training provided for any employees. Manager unable to locate any proof of employee food handler training due to computer access issues.

Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Vacuum breaker missing at the end of the splitter for mop sink faucet where red hose is attached.
