Dead roaches on premises. Upon inspection, observed one dead roach in dining area, one dead roach on floor behind soda machine near drive-thru window and one dead roach on floor in dry storage area. During inspection, manager/employees cleaned up dead roaches and sanitized the areas.

Presence of insects, rodents, or other pests. Three undetermined dead insects observed behind tea brewing station. Operator cleaned and sanitized area during inspection.
Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking. For cut lettuce and tomatoes. Time out of temperature control was 30 minutes. Operator added time mark to items during inspection.
Outer openings not protected during operation and vermin and/or environmental cross contamination present. Back door open during inspection. 3 flying insects present during inspection.
