Raw animal foods not properly separated from one another based upon minimum required cooking temperature when stored in a freezer - not all products commercially packaged. Rebagged portions of frozen raw hamburger meat stored directly on top of bags containing frozen cheese curds on shelf inside walk-in freezer. An employee removed the raw hamburger meat from the shelf during this inspection.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Backflow prevention device missing from red hose connection after splitter added to mop sink.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Ceiling tile at cookline soiled with dust.

Employee touched bare body part and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Male employee wearing gloves touched face and adjusted glasses and continued working with food and single service, Inspector addressed situation, employee stopped removed gloves and wash his hands. Also another male employee adjusted glasses with gloved hands no hand washed, Manager addressed situation.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Reach-in cooler at the cook line raw bacon 52F, Manager moved bacon to freezer.
Equipment and utensils not properly air-dried - wet nesting. Cleaned stacked containers, Manager moved to triple sink.

Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. At the cook line, chicken nuggets 127F, timer not working, nuggets place in drawer around 10:20am Manager voluntarily discarded. Nuggets are good for 1hr per restaurant policy.
