Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Main kitchen prep table- observed employee wipe prep table with wiping cloth, grab case of iceberg lettuce and proceeded to cut lettuce without changing gloves or washing hands. Operator instructed employee to wash hands and change gloves.
