Observed house made pico, cut lettuce, cooked beef (48F-50F - Cold Holding) in reach-in cooler next to triple sink. Operator stated they had the lid opened and had a couple of items pulled out during lunch rush. Operator stated they will keep lid closed to chill foods.

Food-contact surface soiled with food debris, mold-like substance or slime. Observed cutting board on reach-in cooler next to triple sink soiled/ stained.
Mobile food dispensing vehicle disposing liquid waste improperly. Observed a portable waste water receptacle connected by pipe placed in receptacle from MFDV. Observed waste water in portable receptacle. Operator stated they roll portable waste receptacle to commissary across parking lot to empty. Operator uses receptacle as an overflow waste container to avoid moving MFDV. Discussed with operator that the MFDV must be able to move to empty waste or have a service that comes and empty's waste from MFDV but it cannot be rolled across parking lot and waste receptacle needs to be an intregal part of the food truck
No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at hand wash sink. Operator replenished paper towels.
Food stored on floor. Observed plastic bottles of water and a container of oil stored on floor next to fryers. Operator removed items from floor to properly store.
Mobile food dispensing vehicle license number not permanently affixed on the side of the unit in figures at least 2 inches high and in contrasting colors from the background of the vehicle. Observed MFDV having old license # on exterior from previous owner.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed gasket on reach in freezer soiled.
Raw fruits/vegetables not washed prior to preparation. Observed cut avocados not washed prior to prepping. Avocados have sticker still adhered to it.
Time/temperature control for safety food thawed in an improper manner. Observed frozen meat thawing in triple sink without water running. Also, tripe thawing in a pan of standing water. Operator emptied water from pan and turned on running water for all meats in triple sink.
