No probe thermometer provided to measure temperature of food products. Operator left probe thermometer at home, needs to be on site prior to operation.
Bathroom located inside establishment not completely enclosed with tight-fitting, self-closing doors. Restroom requires self closing device.
Hole in or other damage to wall. Walls have numerous holes above tv menus and in restroom. Plumbing openings at handwash sink in back needs to be closed up.
No conspicuously located ambient air temperature thermometer in holding unit. Two reach in freezer and one reach in cooler require ambient thermometer installed.

Outer openings not protected with self-closing doors. Exterior door requires the addition of a self closing device.
Outer openings of establishment cannot be properly sealed when not in operation. Back door has two drilled holes above door knob, approximately 1/4" in diameter.
