Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee came from outside put the gloves on without washing his hands, and started wrapping beef to stored it inside walk in cooler. Employee was coached and washed his hands.
Spray bottle containing toxic substance not labeled. Observed chemical product inside plastic spray bottle with no label. Employee discarded it.
Hole in or other damage to wall. Observed a big hole in the wall behind the entrance door to warewashing area.
Standing water or very slow draining water in handwash sink, three-compartment sink, or mop sink. Observed very slow draining in the hand sink located at the fish processing area.

Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Observed raw chicken sausage above raw pork inside walk-in cooler. Employee inverted the storage order of the products.

Expired chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Quaternary test kit already expired since 11/1/2023. Manager replaced it for a current test kit.
Floor area(s) covered with standing water. Accumulation of water on the floor ( fish prep area).
Wet mop not stored in a manner to allow the mop to dry. Employee corrected it.
