Single-use gloves not changed as needed after changing tasks or when damaged or soiled. During inspection observed cook at cook line put gloves on without washing hands and started opening a bag and touched plantains. Educated employee employee washed hands changed gloves.
Food stored in undrained ice. During inspection observed salmon in undrained ice in reach in stand alone fridge. Educated took all ice out.
