Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Raw fish. Chef took the fish out the package and run under running water.

Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Hose Bibb next outside mop sink has a hose connected to Bibb without a vacuum breaker. Manager put on a vacuum breaker on hose Bibb that is connected to hose.
Food storage container/container lid cracked or broken. Observed in 2 door reach in cooler on cook line had a food container fill with raw scallops that is broken. Manager change out container and discarded the broken container
