Employee dried hands on clothes/apron/soiled towel after washing. Observed employee rinsing their hands with water and no soap at the 3 compartment sink then drying their hands with a soiled towel.
Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed employee touching with toasted bagel and slice tomatoes with bare hands. Inspector asked employee to wash hands and place gloves on before preparing any time of food.
Employee washed hands with no soap. Observed employee rinsing their hands with water and no soap at the 3 compartment sink then drying their hands with a soiled towel.

Observed cooked chicken (49F - Cold Holding); ground beef (48F - Cold Holding); cooked vegetables (49F - Cold Holding); sweet plantains (49F - Cold Holding); white rice (51F - Cold Holding); cooked chicken with sauce (49F - Cold Holding) stored inside the clear door reach in cooler since 5 days ago, as per operator. Observed slice cheese (60F - Cold Holding); slice ham (59F - Cold Holding) inside the metal reach in cooler 1 hour before inspection, advise operator to place items into another reach in cooler. Observed slice tomatoes (47F - Cold Holding); cut lettuce (45F - Cold Holding); slice ham (48F - Cold Holding) stored inside the clear reach in cooler for less than 1 hour, as per operator. Placed items into another reach in cooler.
