Operator and employee stated the tomatoes were cut from an ambient state two hours ago.

Observed rice in container on shelf above bus tub of cut tomatoes, shredded cheese, and housemade salsa kept at room temperature. The operator and employees stated the items have been out for one to two hours and that they are held on time. Discussed proper time marking for each individual item on time.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed Steak (128F - Hot Holding at 1:30 PM); Pork (131F - Hot Holding at 1:30 PM) in steam table. Employee turned down steam table earlier and did not turn it back up. Employee turned steam table up. Items reheated to 165F.
Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Observed sanitizer wiping cloth solution with chlorine at 200+ ppm. Employee corrected to 100ppm.
Food-contact surface soiled with food debris, mold-like substance or slime. Chip dispensing unit has an accumulation of grease and food debris inside of dispenser.
Handwash sink not accessible for employee use at all times. Observed handwashing sink blocked with a cutting board at time of inspection. Employee removed to make handwash sink accessible.

Mobile food dispensing vehicle service opening larger than necessary for food service. Windows on truck were left open with screen doors open. Operator has parts to repair screen window.
