Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Inside cooler; tumbleweeds, guacamole 44F; ( - Cold Holding) Person in charge said items in cooler 1 hr. Placed in walk in cooler. Turned cooler down to 36F.

Placed on time control during inspection. Queso on line at 126F. Person in charge reheated to above 165F.

Bowl or other container with no handle used to dispense food. 2oz portion cup used as scoop in dry spices. Person in charge removed.

Cooked/heated time/temperature control for safety food not cooled from 135 degrees Fahrenheit to 70 degrees Fahrenheit within 2 hours. rice x 2 started at 1400 (80F - Cooling) 1550 immediately asked person in charge to place rice in ice bath to rapidly chill past 70F. Pans were uncovered 2 inch deep and on speed rack no obvious indications why cooling was not being met. Retested rice at 69F-70F by 1600.
Handwash sink not accessible for employee use at all times. Observed pans and lids in hand washing station next to bag n box. person in charge removed items from hand washing station.
In-use utensil not stored with handle above the top of time/temperature control for safety food and rim of the container. Observed shredded cheese scoop handle touching food in flip-top cooler. Person in charge removed handle from food.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed splash zone next to nozzles on soda fountain machine soiled with black/green mold like substance.
Old food stuck to clean dishware/utensils. Observed clear food containers on drying rack with old food stuck to container.
