Employee with artificial nails/nail polish working with exposed food without wearing intact gloves. Bartender wearing long polished nail engaged in food preparation and not wearing gloves, she was portioning a bowl of salsa with ladle and scooping chips wearing finger nail polish on long nails also wearing a bracelet.
Water with a temperature of least 85 degrees Fahrenheit not provided/shut off at employee handwash sink. Sink inside kitchen entry 77F.

Raw animal foods not properly separated from one another based upon minimum required cooking temperature when stored in a freezer - not all products commercially packaged. Raw chicken stored over raw beef in walk in freezer. Manager had employee correct rotation of beef and chicken.

Equipment in poor repair. Ice machine in disrepair. Manager stated a work order has been place and a new ice machine has been ordered.
Floors not maintained smooth and durable in walk in cooler/freezer. Floor has cuts in metal in walk in freezer and both walk in cooler/freezer floors shift up and down when walked on.
