Ceiling tile missing. In dining room ceiling tiles missing due to water damage. Manager states the ceiling tiles are being replaced once leak is fixed.
Equipment in poor repair. Drink machine in server area is in poor repair wood is missing under counter near lip of ice bin. Manager states they are having counter and machine replaced.

Warewashing sanitizing solution exceeding the maximum concentration allowed. Observed three-compartment sink quaternary sanitizer solution at 500ppm. Manager added plain water to solution and retested at 200ppm.
Ice buildup in reach-in freezer and/or walk-in freezer. Observed buildup of ice inside of walk-in freezer on floor.
Wet mop not stored in a manner to allow the mop to dry. Observed wet mop stored in mop sink. Manager hung mops to dry at time of inspection.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed cheddar cheese in walk-in cooler at 45F. Per employee, cheese was out of refrigerator storage for prep and recently placed back inside walk-in cooler.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed hot holding chicken tenders 113F. Employee put tenders back in fryer and reheated to 183F.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed no vacuum breaker on hose near exterior back door.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed residue buildup on ice scoop. Manager placed scoop in dish area for washing.
Lack of toilet tissue at each toilet. Observed no toilet paper in ladies restroom. Manager restocked toilet paper at time of inspection.
Manager or person in charge lacking proof of food manager certification and no other certified food service manager employed at this location. Observed no proof for Manager Howard Tisdale at time of inspection and no proof for any other manager.
Paper towel dispenser at handwash sink not working/unable to dispense paper towels. Observed paper towel dispenser at hand wash sink in kitchen unable to dispense. Manager placed roll of paper towels by sink during inspection.
Floor area(s) covered with standing water. Observed standing water on floor in dry storage area underneath soda syrups.
In-use ice scoop stored on soiled surface between uses. Observed ice scoop stored on top of dusty ice machine. Manger placed scoop in dish area for washing.
