Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Dish machine reading 0ppm chlorine after multiple attempts. Advised employees to use sanitizer sink for all items coming out of dish machine until machine repaired.
Employee handled soiled equipment or utensils and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee handled personal beverage container and continued to portioned fries without washing hands and changing gloves first. Reviewed proper handwashing procedures and employee washed hands and put on new gloves.
Food-contact surface soiled with food debris, mold-like substance or slime. Milk steaming wand on espresso machine has dried milk residue. Employee cleaned wand.
Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory. Tuna and salmon tartare on menus not marked as raw items. Employee began to mark menus to indicate raw product.
