Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory. Observed Tuna curdo and Wagyu carpaccio on the menu. As per the manager those items are served raw. The manager put an asterisk next to the items on the menu with a pen.

Observed the dish washer handle dirty dishes and remove clean dishes from the dish machine without washing his hands.

Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed the prep cook removed gloves after cutting onions and put on a new glove without hand washing.
Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed the prep cook sign a paper with a glove on and with the same glove cut onions.
