Equipment and utensils not properly air-dried - wet nesting. Observed multiple wet containers stack at dish shelf with no airflow. Operator separated the containers to allow airflow.
Unsealed concrete floor in food preparation, food storage, warewashing area or bathroom. Observed unsealed concrete at walk in cooler and ice machine area.

Wet wiping cloths stored in detergent and sanitizer mixed together. Observed wet wiping cloth in detergent and quaternary sanitizer mixed together. Operator discarded the solution of detergent and sanitizer and placed a new container of only sanitizer.

Handwash sink not accessible for employee use at all times. Observed large empty sanitizer container inside the basin of handwash sink next to 3 compartment sink. Operator removed the container.
No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at bar handwashing sink. Operator placed paper towels at handwash sink.

Observed raw beef over cooked ribs at make table with partial barrier in between. Operator removed the cooked ribs from underneath the raw beef.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed residue buildup on can opener blade at prep table. Operator cleaned and sanitized the blade.

Observed fish thawing under cold water at prep sink without being removed from reduced oxygen packaging. Operator removed the fish from packaging.

Observed at 0ppm at bar Dish Machine due to empty sanitizer container. Operator replaced the empty container with a full one. Corrected to 100ppm.

Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Operator printed out the vomit and diarrhea written procedure.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Operator completed the documents during inspection.
Carbon dioxide/helium tanks not adequately secured. Operator secured the tanks.
Wiping cloth sanitizing solution stored on the floor. Observed at kitchen next to wait station. Operator removed the sanitizer off the floor by replacing it with a new container and placed on shelf.
