No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. One new employee working in kitchen handling food without proof of food handler training or proof they have been informed of their health reporting responsibilities. Inspector provided manager a new copy of the employee health reporting agreement during this inspection.
Covered waste receptacle not provided in women's bathroom. Trash can uncovered inside unisex restroom.

Unwashed fruits/vegetables stored with ready-to-eat food. Boxes containing unwashed mushrooms and unwashed tomatoes on shelf above pans of dough and plastic bottles containing soda. Manager moved produce to the bottom shelf during this inspection.

Food stored on floor. Container of cooking oil stored directly on floor in kitchen near handwash sink. Employee placed oil on shelf during this inspection.
Interior of oven/microwave has accumulation of black substance/grease/food debris. Interior walls of microwave at cookline soiled with food debris.
Single-service articles improperly stored. Several boxes of takeout containers stored directly on floor below pizza oven. Manager removed all from floor during this inspection.
Storage of maintenance equipment/tools in areas that may result in cross contamination of food, equipment, utensils, linens and/or single-service and single-use articles. Power drill and battery charger stored on prep table in kitchen beside fryers. Manager moved both to blow front counter during this inspection.
Wall soiled with accumulated grease, food debris, and/or dust. All in kitchen above supply shelf soiled with dust.

Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee handled personal phone, then put on gloves and began working with dough. Manager had employee wash hands and put on new gloves.
Employee personal food not properly identified and segregated from food to be served to the public. Employee personal pot of chili and bowl of rice stored above customer cans of soda, bottled water, and onions inside reach-in cooler at front counter.
Unwashed fruits/vegetables stored with ready-to-eat food. Boxes containing unwashed mushrooms and unwashed tomatoes stored on shelf above ranch dressing and cooked chicken inside reach-in cooler at front counter. Manager moved unwashed produce to the bottom shelf during this inspection.
