Employee used gloved hands to pull up shirt at the shoulders and continued food preparation without washing hands and changing gloves. Manager stopped employee and had him wash hands and change gloves.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. No vacuum breaker after "Y" fitting at mop sink on hose side. Manager installed vacuum breaker during inspection.
