Observed cooked mushrooms (114F - Hot Holding); cooked peppers (109F - Hot Holding) in front line steam table. As per operator, items were placed in steam table approximately 10 minutes ago. Operator increased the temperature on the steam table and brought the item back to 142F.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed at mop sink a splitter on the hose bibb missing a vacuum breaker.
Floors not maintained smooth and durable. Observed floor in walk in freezer not smooth or durable.
Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed microwave near the take out window with accumulation of food debris and grease. Operator cleaned properly.

Reviewed proper reheating for hot holding procedures with operator. Operator put cheese sauce on grill to reheat to 165°F or above. Reheated to 167°F

Employee with no beard guard/restraint while engaging in food preparation. Observed cook engaging in food prep at front counter, no beard guard. Cook put beard guard on.
