Per manager, all products have been in the same cooler overnight.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. See stop sale. Observed wings (68F - Cold Holding); cheese sticks (63F - Cold Holding); cheese (54F/55F - Cold Holding) in reach in cooler by fryers. Per manager, all placed in cooler approximately 3.5 hours prior. Wings and cheese moved to reach in freezer to quickly chill. Also, observed cut tomato (67F - Cold Holding); cut lettuce (61F - Cold Holding); turkey (67F - Cold Holding)in reach in cooler by fryers. Per manager, all products placed in cooler in the last 1.5 hours. Products moved to reach in freezer to quickly chill. Raw animal food stored over/not properly separated from ready-to-eat food. Opened case of raw bacon stored over cut lettuce in far left reach in cooler. Manager rearranged to proper storage.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed chicken at 52F and wings at 51F in flip top reach in cooler in kitchen. Chicken brought from reach in cooler approximately one hour prior and wings brought from reach in cooler approximately 2.5 hours prior per manager. Manager removed both products and put into another cold holding unit. Ranch dressing on counter beside fryers at 82F. Manager states dressing was left out for approximately 45 minutes. Dressing placed in reach in freezer and approximately 20 minutes later, temperature is 48F. Cheese in small flip top reach in cooler at 49F. Observed cheese stored above pan chill line. Manager states product was brought from another cooler approximately 30 minutes prior. Manager removed cheese to reach in freezer and approximately 20 minutes later temperature is 39F.

No proof of required state approved employee training provided for employee hired more than 60 days ago. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. No proof of food handler training for Crisha D, hired over one year ago, per manager. All training certificates missing trainer certification number. All training certificates missing employee's last names. Two training certificates missing dates of birth. Multiple training certificates have incorrect expiration of five years.
Employee personal food not properly identified and segregated from food to be served to the public. Employee food stored over cut vegetables in reach in cooler. Manager moved employee food. Employee food stored on top of cooked chicken for customers in flip top reach in cooler. Manager moved employee food.
Equipment in poor repair. Handles of doors on flip top reach in cooler have pieces of the handles missing.
Food storage container/container lid cracked or broken. Food storage pans for in use turkey and sliced cheese in kitchen have a piece of container missing on the sides.
