Heat strip failed to turn black to indicate the sanitization temperature of 160 degrees Fahrenheit was achieved on the dish surface and/or high temperature holding thermometer did not reach 160F. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired. Observed hot water sanitizing dish machine at 154F on the final rinse. Operator discontinued use and set up 3 compartment sink to sanitize.
No handwash sink located in vending machine area where employees service bulk food machines. Observed a whisk in the handwashing machine sink on the cook line. Operator removed the whisk.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed interior downstairs ice machine with black mold like substance. Operator was in the process of cleaning ice machine.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed black cod at 50F and beef tenderloin at 48F. Operator stated items had been in drawer on cook line for approximately an hour. Operator put ice packs on items and moved them to a different cooler.
Toxic substance/chemical improperly stored. Observed window cleaner on the shelf next to salad bar station. Operator removed cleaner.

As a repeat violation the importance of following proper protocol with this violation was reinforced with the employee.
