Raw animal food stored over/not properly separated from ready-to-eat food. Observed macaroni and cheese under raw chicken in reach in cooler. Observed Operator removing macaroni and cheese and putting it on top for proper storage.

Buildup of food debris/soil residue on equipment door handles. Observed back sides of reach in handles with food residue on the handles. Observed operator cleaning all reach in door handles.
Cutting board has cut marks and is no longer cleanable. Observed cutting boards with grooves through out boards
