Ready-to-eat, time/temperature control for safety food marked with a date that exceeds 7 days after opening/preparation. Observed cooked burger patties with date marking stating 6/23, day of inspection. Person in charge stated burgers were cooked day prior on 6/22.
Equipment in poor repair. Observed torn gaskets on reach in cooler and reach in freezer doors on cook line.

Raw animal foods not properly separated from one another based upon minimum required cooking temperature when stored in a freezer - not all products commercially packaged. In reach in freezer on cook line, open case of raw beef patties stored on shelf above open bag and pan of raw fish patties. Manager moved raw fish to shelf above raw beef patties.

Unwashed fruits/vegetables stored with ready-to-eat food. In walk-in cooler, unwashed lettuce and tomatoes stored on shelf above ready to eat cut tomatoes. Manager moved prepped tomatoes to different shelf.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In reach in cooler in drive thru area sweet cream (56F - Cooling); cheesecake filling (54F - Cooling). Manager stated they had temped at 9 am and they were below 41. Manager stated they had been in the cooler. Manager placed items in walk-in cooler to bring back down to temperature. Rechecked temperature sweet cream 44F, cheesecake 50F.
No soap provided at handwash sink. At handwash sink by mop sink no hand soap available. Manager replaced soap.
Dead roaches/insects on premises. 3 dead flies under front counter by registers. 1 dead lizard on floor by walk-in cooler. Manager cleaned up all dead pests.
Equipment in poor repair. Multiple gaskets throughout kitchen torn.
Ice buildup in reach-in freezer and/or walk-in freezer. In stand up reach in freezer by fryers on cook line has build up of ice. Also, walk-in freezer alone back wall has ice build up on the floor.

Reach-in freezer interior/shelves have accumulation of soil residues. Reach in freezer on cook line by fryers build up of food debris in bottom.
Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. On cook line Sanitizer Bucket (Quaternary 0ppm). Manager discarded and filled with new Sanitizer Bucket (Quaternary 0ppm). Bag in dispenser was almost empty, manager changed bag.

Proof of required state approved employee training not available for some employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Missing for one employee working today.
Soap dispenser at handwash sink not working/unable to dispense soap. Soap dispenser at hand washing sink near mop sink in kitchen.
Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Two blue ice buckets stored by ice machine. Manager inverted ice buckets.
Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Employee coffee drink cup stored on shelf above make line in kitchen. Drink moved and stored correctly.
Ice buildup in reach-in freezer and/or walk-in freezer. Stand up reach in freezer near cook line.
A return visit. The problems from the previous inspection had been dealt with.
Violations found and a warning issued, with a return visit required. 7 recorded, 4 high priority.

Employee touched face/hair and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee at drink station near drive through window wiping face and handling necklace and bracelets with gloved hands then continued to make drinks without changing gloves and washing hands.
Establishment operating with changed menu and/or increased seating without municipal approval for such change. Establishment licensed for 15 seats, but currently has 20 seats.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. cheesecake mix (53F - Cold Holding) in small reach in cooler near drive through window, per manager mix has bee taken in and out of cooler for less then 1 hour, lid removed from mix., retemp after approximately 40 minutes at 41F.
Toxic substance/chemical improperly stored. Chemical spray bottles with blue and yellow liquid stored hanging on rack facing box of single service utensils, manager removed chemical bottles.
Handwash sink not accessible for employee use at all times. Sanitation buckets in hand wash station by mop sink near office area, buckets removed
No currently certified food service manager on duty with four or more employees engaged in food preparation/handling. A list of accredited food manager certification examination providers can be found at http://www.myfloridalicense.com/DBPR/hotels-restaurants/food-lodging/food-manager/ Manager on duty, Angela Gueye, does not have any proof of manger certification with 4 other employees present during inspection.
Reuse of single-service or single-use articles. Establishment reusing old box for tomatoes to hold sliced cheese.
