Heat strip failed to turn black to indicate the sanitization temperature of 160 degrees Fahrenheit was achieved on the dish surface and/or high temperature holding thermometer did not reach 160F. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired. Dishwasher (Temperature 147F) after running multiple times. Operator switched mode to chlorine sanitizer and called technician, began resanitizing dishware at 50ppm chlorine.

Cardboard used as a food-contact surface. In protein walk-in cooler, cardboard from raw salmon cardboard box in direct contact with salmon, used as label. Operator asked employee to line with single service plastic wrap for proper food protection.
Ice buildup in walk-in freezer.
In-use utensil not stored on a clean portion of food preparation or cooking equipment. On back prep line observed large whisk and paddle stored on top of soiled oven.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. shrimp (56 F - Cold Holding) per operator shrimp was thawed under running water 2 hours prior. Operator iced down shrimp to rapidly chill.
