Handwash sink used for purposes other than handwashing. Observed hand wash sink on cook line used to store a couple of large knives and hand wash sink in bar area has wedger in hand wash sink. Operator removed items from both hand wash sinks.

Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice buildup up on floor, pipe and fan in walk in freezer. Operator chipped ice away during this inspection.
Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed interior of microwaves on cook line and interior of ovens in prep area and turbo oven on cook line soiled with accumulation of debris/ black substance.

Also, interior of hand wash sink on cook line soiled. Also, exterior of equipment on cook line and prep area (fryers, reach-in coolers, drawers, bump bars and cords for equipment, etc) soiled. Also, exterior of multiple seasoning bins in prep area and dry food storage area soiled. Also, exterior of paper towel dispenser on cook line near breading station soiled. Also, filters in hood above fryers and grill soiled.
Old labels stuck to food containers after cleaning. Observed multiple clean pans on storage shelves in warewashing area still have old label adhered to pan/ container.
Plumbing system in disrepair. Observed hand wash sink on front service line covered with do not use sign. Operator stated they are waiting on a part to repair hand wash sink.

Discussed with operator that same foods in back of drawer in temp, but those in front of drawer near opening were out of temperature, the drawer may not have been fully closed during rush. Operator placed bags of ice on both products to chill.
Employee with artificial nails/nail polish working with exposed food without wearing intact gloves. Servers pulling plated food at expo area wearing nail polish without wearing intact gloves.
Food-contact surface soiled with food debris, mold-like substance or slime. Underside of lemonade machine at beverage station near front service line and near bar area, underside of mixer in prep area, soda gun in bar area, and interior of chip machine on front service line soiled.
Required employee training expired for some employees. Two employees (Emily, Sarymar) food safety certification expired 2024. Operator stated they are working on completion.

Employee with ineffective hair restraint while engaging in food preparation. Multiple employees pulling food at expo line, changing out soda boxes, carrying trays of plated food, and making beverages have ineffective hair restraint.
Equipment in poor repair. Cutting board at reach-in cooler across from breading station chipped at corner. Also, green lid on seasoning in prep area chipped/ cracked.

Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Sanitizer Bucket (Quaternary 0ppm); on cook line, and front service line. Observed server had just made solution, tested at 0ppm. Observed bottle of sanitizer at triple sink empty. Discussed with operator about empty container of quaternary solution at triple sink. Operator changed out container and remade solution, tested at 200ppm.
A return visit. The problems from the previous inspection had been dealt with.

Two different servers placed on gloves after returning from the dining room, and began to remove food from pass through window without washing hands. Manager coached employee on proper hand washing procedures. Employee washed hands correctly.
Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. In cooler drawers below grill on cook line in kitchen, pan of raw ground beef stored over pans of raw in-tact beef steaks. Items moved and stored correctly.

Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Breaker missing at the END of the splitter for mop sink faucet where blue hose is attached. Splitter removed and blue hose attached directly to breaker on mop sink faucet.

Manager removed all TCS foods from bottom of cooler and ice bath the top of the top cooler. Observed pico (50F - Cold Holding); Pineapple pico (50F - Cold Holding); Sour cream (49F - Cold Holding); Butter (49F - Cold Holding) at flip top cooler near reheat station. Manager found out that cooler had not been turned on.
